Please return in the stamped envelope, together with full payment to: DNA QLD PO BOX 1955 MILTON BC Q 4064 Step 2:Once payment has been received, DNA QLD staff will contact each person (or their solicitor) to coordinate the sample collection. Sample collection has to be done by a collector authorised by DNA QLD and may take place at a Medical Practitioner, a Pathology Collection centre, DNA QLD collection facility or via other arrangements suitable under the Australian Family Law Courts. PLEASE NOTE: - The cost of the medical practitioner’s consultation fee for conducting DNA collection is NOT included in the cost of paternity testing and MUST BE MET BY THE CLIENT. DNA QLD will mail the legal affavidit, Form 2, to your home. Organise 2 passport sized photographs for each person being tested before the day of collection. Each person who is having a sample collected must take with them the completed FORM 2 and two passport-sized photograph, with a full view of head and shoulders against a plain background. Separate photographs are required for each person. One photo is required for the Form 2 and the other photo for Form 4. Step 3: On the day of sample collectionForm 2 must be completed and witnessed by a Justice of the Peace, Solicitor or Police Officer on the day and BEFORE you have the sample collection. The collection is legally invalid if the Form 2 or the collection is not done correctly. The sampler needs to site formal identification for each individual having swab collection and note on the form. This ID may be a driver's license number, 18+ card number, Medicare number, birth certificate or passport . Newborns may be identified by their hospital armband or crib card. Do not eat or drink for 30 minutes prior to the swab collection. Nursing mothers should also not feed babies for 30 minutes before the collection. The person collecting your sample will complete FORM 4 (sent to you by DNA QLD), by attaching your photographs and signing them, so that it is evident in the event if they have been tampered with. Form 2 and Form 4, the indeminity form, the swabs are returned to DNA Qld in the supplied Australia Post satchel.
Step 4:The testing procedure normally takes approximately 5 - 7 working days. Results will be forwarded to the client or the client’s solicitors by mail. Results will not be given over the phone under any circumstances. DNA QLD ensures strict privacy and confidentiality of clients undergoing paternity testing. Many issues involving paternity testing can be highly emotional. Counseling prior to, and following testing is recommended. DNA QLD can provide the name of a Clinical Psychologist if requested.
