INSTRUCTIONS FOR "PEACE OF MIND" OR HOME DNA TESTING

 

Step 1:

Download and complete the application form and return with payment to DNA QLD

Step 2:

Once payment has been received, DNA QLD staff will mail the home DNA collection kit to the nominated address or addresses. Contact will be made with the applicant/applicants if requested on the application form, to arrange the DNA collection. Collections made be done by yourself at home or you may prefer to consult your medical practitioner for the swab collection. DNA QLD provide a free collection service in Brisbane if requested.

 

Step 3:

Do not eat or drink or smoke for 30 minutes prior to the swab collection. Nursing mothers should also not feed babies for 30 minutes before the collection.

The person providing the sample will complete FORM 4 (sent by DNA QLD). The sampler needs to write their name, address, time and place of collection on the top of the form. The sampler needs to sign and date the bottom of the form.

The sampler needs to site formal identification for each individual having swab collection and note on the form. This ID may be a driver's license number, 18+ card number, Medicare number, birth certificate or passport . Newborns may be identified by their hospital armband or crib card.

Collect the samples as per the printed instructions or the video instruction on the web page. Place the labelled samples and paperwork in the supplied envelope and post immediately to DNA QLD in the supplied Australia Post satchel .

 

Step 4:

The testing procedure in the DNA testing laboratory normally takes approx. 5 -7 working days.  Results will be forwarded to the client’s home address via registered postage. Results will not be given over the phone under any circumstances. 

Please note that results from this testing procedure are not admissible in the Family Law Court. Please contact us if your case requires Family Law Court evidence.

Many issues involving paternity testing can be highly emotional. Counseling prior to, and following testing is recommended. DNA QLD can provide the name of a Clinical Psychologist if requested.
